Very tough case with a lot going on, but I think we achieved an exceptional result. This patient presented with an implant placed and temporized by another local doctor with the complaint that she wasn’t happy with her gum line and tooth shade (which is totally normal in this stage). She also presented with a fracture of her adjacent front tooth.

What made this case complicated was a mismatch in spacing between her front teeth that if restored as is, would have led to a gap between her front teeth, or one front tooth being larger than the other.

To combat this, I placed a composite veneer to close the diastema/increase the width of the tooth. Afterwards, I had my expert dental implant lab @prosmilesdentalstudio fabricate a ZR-Ti hybrid custom abutment with a layered zirconia crown.

When altering the gum line, it’s important to have good control over the subgingival contours of your custom abutment and crown. I think the interproximal gingival contours will continue to improve over the next few months to help further eliminate black triangles.

The end result was a very happy patient who can confidently smile again!

In this case, we accomplished several goals: Removal of decay around the existing teeth, improved shade, improved position of the dental midline relative to the facial midline, and improved vertical dimension and occlusion (bite)

In addition to this work, we removed a failing bridge, while being able to spare the anterior abutment, on the lower left and placed an immediate implant for tooth #19. This will be ready for a restoration in 2 months.

Esthetic Zone Crown Lengthening BioTemp Phase

This case shows what can happen when dentistry is performed poorly. This patient presented with severely inflamed gingiva as a result of biologic width impingement and poorly sealed crowns allowing a bacterial invasion. (X-rays at the end are pre-treatment).

Upon removing the old dental work, I performed biologic shaping (removing the old margins), laser gingivectomy, and crown lengthening (bone removal) to create an esthetic gingival display as well as a healthy environment for our new dental work.

After an initial healing period of 4 weeks, I digitally scanned the teeth to have long term, individual temporary crowns (BioTemps) made. By fabricating individual crowns instead of splinted crowns, the patient will be able to brush and floss effectively in order to allow the gums to fully heal, before proceeding to final restorations.

After another 6-8 weeks of healing, we will proceed to have the final crowns made (as long as all gingival inflammation is gone, and the gingival papilla is filled in).
